Intake Forms
Intake forms let you collect important information from clients before their appointment โ health history, preferences, consent, or anything else you need to know.
How do I create an intake form?

Go to Intake Form in the sidebar

Click + Add Question

Choose the question type: text, multiple choice, yes/no, or date

Mark questions as required if needed

Save the form
The intake form appears on Step 3 of your booking page, right after the client selects a time slot.
Can I make fields required?
Yes โ when adding a question, toggle the Required option. Clients cannot complete the booking without answering required questions.
Where do I see form responses?
Intake form responses are saved with each booking. To view them:

Go to the Bookings tab

Click any booking

Scroll to the Intake Form Responses section
Use case: A massage therapist asks about injuries or areas to avoid. A personal trainer asks about fitness goals. A medical spa asks about allergies.
What is the difference between intake forms and agreements?
- Intake forms โ collect information (questions and answers)
- Sign Agreements โ legal documents clients sign with their digital signature (available on Premium plan)
